Specimen Requirements
All three of the following specimens are required:
Specimen Type: Whole blood
Container/Tube: Pink top preferred (K2 EDTA) or Purple top
Specimen Volume: 6 mL
Specimen Minimum Volume: 3 mL
Collection Instructions:
1. Properly identify patient and draw specimen post-transfusion reaction. Do not reband patient.
2. Label specimen as follows prior to leaving patient’s side as follows:
a. Patient’s full name, DOB, and medical record number (demographic label is acceptable)
b. Date and time of draw
c. Phlebotomist’s initials
d. Band number sticker from the existing blood bank wristband.
3. Do not centrifuge. Forward unprocessed whole blood promptly.
Specimen Rejection:
Proper labeling is critical as this specimen will be used to identify any clerical errors.

Container/Tube: Red top or gold top
Specimen Volume: 3 mL
Specimen Minimum Volume: 1 mL
Collection Instructions: Specimen must be collected post-transfusion reaction. Properly label specimen and forward to blood bank promptly.
Specimen Type: Urine
Container/Tube: Clean container
Specimen Volume: 10 mL
Specimen Minimum Volume: 3 mL
Collection Instructions: Collect a random urine collection post-transfusion reaction. Properly label (demographic label acceptable) and send to blood bank promptly.
