Specimen Requirements
Specimen Type: Semen
Collection Container/Tube: Sterile container
Submission Container/Tube: Plastic container
Specimen Volume: Total ejaculate
Collection Instructions:
- Patient should have 2 to 5 days of sexual abstinence.
- Entire ejaculate is best collected by masturbation. (A special condom [supplied by physician] is acceptable.)
- Label container with patient’s name (first and last) and date and actual time of collection.
- Maintain specimen at body temperature (37° C) and forward promptly.
- Complete special collection instructions below and send form with specimen.
